The Haitian Adoption Process (for Americans)—The Dossier (part two)

What is needed to complete a Haitian adoption dossier, continued from Part One:

• Positive Home Study: A state-licensed agency or social worker must do the home study. The agency and/or social worker should also include a copy of their license. Please request that it be no more than 6-8 pages in length. You need TWO Notarized originals. One for your dossier and one for your I600a which you or your social worker may send. (Notarized & State Authenticated.)
• Psychological letter: This letter must contain specific results from a MMPI II (or newer version) test. The letter must also address whether the psychologist/psychiatrist finds the client mentally and emotionally stable and capable of handling more children. The letter may address both parents in one letter, but must give specific MMPI results on each client. (Notarized & State Authenticated.)
• Letter of Good Conduct (one per adoptive parent): This letter should be obtained from the client’s local police office (usually from the records department). The letter must state that the client is a citizen in good standing. It should state how long the parent has lived in the county and that there are no criminal history records on file or current warrants out for his/her arrest. (Notarized by police station.)

• Medical Letters: Each parent must have a medical letter. The letter must specifically state that the individual has been tested for HIV, VDRL and is free of any communicable diseases and is physically capable of raising adopted children. The patient must also have a complete CBC done as well. It is helpful to include how long each client has been a patient of the doctor’s. The medical letter must be done on the medical office’s letterhead. (Notarized by Doctor.)
• Last three years of tax returns: Only the federal tax returns are required. The first two pages of the return and Schedule A, if they itemized deductions, are sufficient. W-2’s and 1099’s are not required or needed.
• 3 Letters of Personal Reference: These letters must not be from relations. One letter should be from a religious leader. Try to keep under one page. (Notarized by reference.)
• CIS Form G-28: http://www.uscis.gov/files/form/g-28.pdf (Download this form and print it. Fill in the top 3 boxes. In Re: put name and then _____ family adoption; date; name; mark the "Petitioner" box; fill in address and then put "adoption" in the big box right above where you will print, sign, and date the form. (Must be Notarized,)
• CIS Form I-600. Both parents, if applicable, must sign this. Download from the following website: http://www.uscis.gov/files/form/i-600.pdf (Must be Notarized.)

The dossier should be photocopied. You will need to ask your agency how many copies they require, and you will want to keep a copy for your own records.
